Parent/Caregiver that DOES Receive Assistance from a Human Service Agency:

If you are a parent or caregiver and you currently receive assistance through any Bucks County Human Service agency, your caseworker must make the request on your behalf. Ask your caseworker to fill out the form below.

Parent/Caregiver that does NOT Receive Assistance from a Human Service Agency:

If you are a parent or caregiver and you do not currently receive assistance through any agency, you will need fill out the form below and also upload one form of documentation.

Your documentation must be:

  • A copy of child’s Free or Reduced Lunch Form – or –
  • A copy of your Access Card and valid Driver’s License – or –
  • A very brief email from the school nurse, guidance counselor or social worker stating that you are a family in need and could benefit from Gift Cards. (Have them email this to Danielle Bush at
Parents without a caseworker must fill out the form completely and accurately – in case we need to contact you.
Caseworker or School Representative:

If you are a caseworker, agency or school district representative, please fill out the form below.

**Please note: This program is exclusively for families with a significant need. You will be asked upon completion of this form to upload a list of recipients. Please list the following: child(ren)'s first name OR first initial and last name, sex (m/f), school district, grade level, and age.
