Parent/Caregiver that DOES Receive Assistance from a Human Service Agency
If you are a parent or caregiver and you currently receive assistance through any Bucks County Human Service agency, your caseworker must make the request on your behalf. Ask your caseworker to email your child or children’s age, grade, and gender to Danielle at danielleb@uwbucks.org.

Parent/Caregiver that does NOT Receive Assistance from a Human Service Agency
If you are a parent or caregiver and you do not currently receive assistance through any agency, you will need to email your child or children’s age, grade, and gender PLUS one form of documentation to danielleb@uwbucks.org.

Your documentation must be:
· A copy of child’s FREE or Reduced Lunch Form – or –
· A copy of your Access Card and valid Driver’s License – or-
· A very brief email from the school nurse, guidance counselor or social worker stating that you are a family in need and could benefit from school supplies.
Parents without a case worker must include complete contact information – name, address, and a working phone number in case we need to contact you.

Caseworker or School Representative
If you are a caseworker, agency or school district representative, email your request for backpacks to danielleb@uwbucks.org. The request must be broken down by both grade level – elementary, middle, or high school – and by gender, boy or girl. We need to know how many backpacks you need in each category.
